The investigative committee established to probe the August 6, 2025, military helicopter crash has submitted its final report to President John Dramani Mahama.
The report was formally presented by the committee’s chairperson, Acting Minister for Defence Dr. Cassiel Ato Forson, during a National Security Council meeting at the Jubilee House in Accra on Monday, November 10.

Formed shortly after the tragedy, the committee was mandated to determine the cause of the crash and propose preventive measures to avert future incidents. Its comprehensive report reportedly outlines technical, operational, and environmental factors that may have contributed to the disaster.
According to government insiders, the findings will be made public on Tuesday, November 11, 2025, during an official press briefing where key recommendations and the government’s next steps will be announced.
The fatal incident involved a Harbin Z-9EH military helicopter (tail number GHF 631) belonging to the Ghana Air Force. The aircraft took off from Accra around 9:12 a.m., bound for Obuasi in the Ashanti Region for an anti-galamsey operation, before losing radar contact and crashing in the Adansi Akrofuom District.

All eight passengers and crew members aboard the helicopter perished in the crash. The victims were identified as Dr. Edward Omane Boamah (Minister for Defence), Alhaji Dr. Ibrahim Murtala Mohammed (Minister for Environment, Science and Technology), Alhaji Muniru Mohammed Limuna (Acting Deputy National Security Coordinator), Dr. Samuel Sarpong (NDC Vice-Chair), Mr. Samuel Aboagye (former parliamentary candidate), Squadron Leader Peter Bafemi Anala, Flying Officer Manaen Twum Ampadu, and Sergeant Ernest Addo Mensah.
The investigation—chaired by National Security Coordinator Abdul-Osman Razak—was carried out in collaboration with the Ghana Air Force, the Ghana Civil Aviation Authority (GCAA), and international aviation experts.
